Within the project CoCa (Continuity of Care) and related activities the network “Continuity of Care in the Osnabrück Region” which is coordinated by the University of Applied Sciences, explores the possibilities of establishing an electronic discharge summary between different types of healthcare providers. The activities embrace the analysis of current practice and the development and evaluation of a software prototype. The prototype makes use of the ICNP as a reference terminology mapped to other terminologies serving as front end terminologies.
