  • Abstract
    We are currently working on a project that involves identifying the Cardiopulmonary ICF core set for hospice care. As the clinician identifies disability related to the life limiting diagnosis working thru the Hospice Cardiopulmonary ICF core set the ICNP® interventions and outcomes that we have been utilizing are being mapped (I use this term in regards to associating the best interventions that will produce the desired outcome using a nursing group utility mapping process within our database) If the intervention is not utilized variance codes are accessed - to reflect the reason why. It could be the patient refuses, spiritual barrier, cultural barrier and the nurse writes a note as to what is acceptable to the patient/family and researches/adds the second best option if the patient/family wishes to pursue. This project will continue through the primary disease categories that Medicare Local Coverage Determinations are available for. The ICNP® Palliative Care to promote Dignified Dying Catalogue gave us an excellent nursing taxonomy to utilize for this project.
