  • Abstract
    Allina Hospitals & Clinics is a not-for-profit health care delivery organization headquartered in Minneapolis, Minnesota, USA. Allina owns and operates 11 hospitals and 70 ambulatory clinics throughout Minnesota and western Wisconsin. Allina proposes to build nursing care plans for their roughly 200,000 annual inpatient admissions within Epicare Inpatient (Epic Systems Corporation) using ICNP. Epicare IP provides a software infrastructure but not care plan content. Users of the application must purchase or create content. ICNP terms will be visible to end users while the codes will be associated to the terms in the background. The project will be broken into phases. Phase 1 – Create the process and developing a model (May- Dec 2008) The objective for this initial phase is to create a repeatable process and infrastructure in which to develop ICNP based care plans. Existing ICNP catalogs will be used whenever possible. They will be created as a by product of the project where they are not available. Four care plans (pressure ulcer prevention and care, pneumonia, congestive heart failure, and acute myocardial incident) which align with the Allina’s core measures of care will be created during this phase. Support materials such as standards of nursing care, rules and alerts, information links and monitoring reports will be created to support the proper application and use of the care plans. An adaptive design process which involves modeling, feedback from both process and care plan users and adjustment based on feedback will be used to cerate both the process and the product. Phase 2 – Completion of care plans (Jan 2009 –Dec 2010) The objective for this phase is to complete a full set of care plans to be used throughout Allina. Existing ICNP catalogs will continue to be used whenever possible. They will continue to be created as a by product of the project where they are not available. The process and support methodologies such as information links and rules and alerts created in Phase 1 will continue to be employed. Phase 3 – Using the information from the care plans (March 2009-Dec 2010) There are two objectives for this phase. The first is to associate nursing interventions to patient outcomes in the first four care plans (pressure ulcer prevention and care, pneumonia, congestive heart failure, and acute myocardial incident) which align with the Allina’s core measures of care. The second is to experiment with associating measures of time with the interventions to measure nursing productivity.
